Lost or Stolen Passport: DS-64 and DS-11

If your passport was lost or stolen, you must report it to invalidate it before or while applying for a replacement. Name Change: DS-11 vs. DS-5504

Not every name change requires a DS-11. Use Form DS-5504 (Application for a U.S. Passport — Name Change, Data Correction, and Limited Passport) if your passport was issued less than one year ago and your legal name changed within that same year — DS-5504 carries no application fee (only the optional $60 expedited fee applies).
